HSC 119315 -Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATIONS: <br /> Sterilization certificates were not provided for Garrett Hardin's Tattoo Express (XXX) cartridge with the lot#21-12, Daniel <br /> Bryant's Stigma cartridges with the lot#2025Q3, Daniel Bryant's Ambition cartridges with the lot#202309, Anthony Bottom's <br /> Kwadron cartridges with the lot#0518, Teri Eisert's T&T cartridges with the lot#A1214RS or Jazzmin Martinez's Mast Pro <br /> cartridges with the lot#2024-06-01. <br /> CORRECTIVE ACTIONS: <br /> A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, <br /> date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. Provide EHD with the sterilization certificates mentioned above. <br /> 18. HSC 119303-Client records approved and available-Consent form and questionnaire <br /> OBSERVATIONS: <br /> Some of the client records were missing client IDS, lot number(s) of instruments used, name of client and address of client. A <br /> few of the client IDs were not clear and eligible. <br /> CORRECTIVE ACTIONS: <br /> The facility's client consent form and medical questionnaire must include all of the information listed in Form B, and are available <br /> for inspection. Ensure client records are completely filled out, a copy of the client's photo ID is attached and the lot number(s)of <br /> instrument(s)that were used are written or attached to the forms. <br /> 32. HSC 119314-Floors and walls clean and in good repair, adequate light <br /> OBSERVATIONS: <br /> The floor near Daniel Bryant's workstation is damaged. <br /> CORRECTIVE ACTIONS: <br /> A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. Repair <br /> the damaged area of the floor and provide photographic evidence to the EHD. <br /> 37. HSC 119312 - Permits obtained and available <br /> OBSERVATIONS: <br /> Garrett Hardin and Mario Morales did not pay their current permit fees. Invoices were left at the facility. <br /> CORRECTIVE ACTIONS: <br /> A person shall not perform body art without a valid registration and a permitted facility. A body art facility shall notify the LEA in <br /> writing within 30 days of resignation, termination of a practitioner. A new hire or apprentice must be registered with the EHD prior <br /> to performing body art at the facility. Garrett Hardin and Mario Morales are to IMMEDIATELY pay all past due fees.
