• The signatures of all samplers and/or the sampling team leader in the designated <br /> signature block; <br /> • The sampling station number, date, and time of sample collection, and sample <br /> preservation type must be included on each line (each line shall contain only <br /> those samples collected at a specific location); <br /> • The field investigator and subsequent transferee(s) must document the transfer <br /> of the samples listed on the COC in the spaces provided at the bottom of the <br /> form (both the person relinquishing the samples and the person receiving them <br /> must sign the form; provide the date and time that this occurred in the proper <br /> space on the form; and usually, the last person receiving the samples or evidence <br /> should be a laboratory sample custodian); <br /> • The COC is a serialized document. Once the Record is completed, it becomes an <br /> accountable document and must be maintained in the project file. The suitability <br /> of any other form for COC should be evaluated upon its inclusion of all of the <br /> above information in a legible format. <br /> 4.2 Sample Nomenclature <br /> Groundwater, soil vapor, sub-slab vapor, and indoor/outdoor samples will be labeled in <br /> the field using the date collected in a YYYYMMDD (Year, Month, Day) format <br /> followed by the location name (ex. 20211122-MW-6C). Field duplicates will have the <br /> same format as the original sample followed by DUP (ex. 20211122-MW-6C-DUP). <br /> Equipment blank and trip blank samples will be labeled in the field using the date <br /> collected in a YYYYMMDD format followed by EB or TB, respectively, then followed <br /> by a two-digit number designating the sequence the blank was taken(ex. 20211122-TB- <br /> 01). <br /> 4.3 Sample Packajinj! and Handling <br /> As samples are collected in the field, they will be handled and placed into containers as <br /> specified by the laboratory. Each sample label will specify the analyses to be completed <br /> for that particular container(i.e. containers to be analyzed for VOCs by USEPA Method <br /> 8260B will be labeled as such, etc.). A list of appropriate analyses for each sample will <br /> be included on a completed sample COC form for the laboratory. The COCs will <br /> include the sample identification, location, date and time of the sampling, number and <br /> type of containers, and a list of analyses to be performed. Sample documentation will be <br /> completed in the field, and the COCs will accompany the samples through the <br /> transportation and analysis. Samples will be packed in ice chests containing sealed blue <br /> ice or wet ice. Special care will be taken to pack containers to minimize breakage. <br /> Appendix C 12 <br />
