Three <br /> violations were closed from the HMBP inspection.All other violations remain open. Please see the <br /> feedback below: <br /> HMBP: <br /> • Violation #5—Closed <br /> • Violation #10—Closed <br /> • Violation #11 —Closed <br /> • Violation #12—Open. The evacuation plans are not included in the updated Emergency Response <br /> Plan submitted to CERS. <br /> • Violation #13—Open.The training plan submitted does not cover evacuation procedures. <br /> • Violation #14—Open. HMBP training must include the facility's emergency response contingency <br /> plan.The plan submitted to CERS does not cover evacuation procedures (see violation #13). <br /> Please submit training records demonstrating that employees have been provided HMBP training <br /> including the content of an adequate emergency response contingency plan that covers <br /> evacuation procedures. <br /> Hazardous Waste: <br /> • Violation #2—Open. Proof of hazardous waste training has not been submitted. <br /> • Violation #11 —Open. <br /> o Observation 1 -Supporting documentation demonstrating that a complete hazardous <br /> waste determination has been made for each waste stream has not been submitted. <br /> o Observation 2—This has not been addressed. <br /> o Observation 3—This has not been addressed. <br /> • Violation #21 —Open. Disposal records have not been submitted. <br /> • Violation #23—Open.The photo submitted of the used oil drum is missing the accumulation start <br /> date. <br /> Tiered Permitting: <br /> • Violation #18—Open. Proof that the leak was repaired has not been submitted. Additionally, <br /> supporting documentation verifying that all of the requirements for a commercial laundry facility <br /> have been met has not been submitted.
