Assure that the permanent makeup and nail activities are completely separated by a ceiling to floor partition. <br /> 35. HSC 119313—(a)A body art facility shall maintain and follow a written Infection Prevention and Control Plan, <br /> provided by the owner or established by the practitioners, specifying the procedures to achieve compliance with each <br /> applicable requirement of this chapter. (b)The infection prevention and control plan shall include all of the following: <br /> (1) Procedures for cleaning and decontaminating environmental surfaces. (e) Records of training required pursuant to <br /> this section shall be maintained for three years and shall be available for inspection upon request of the enforcement <br /> officer. <br /> OBSERVATIONS: <br /> 1. A copy of the infection prevention control plan was not available on-site for review. <br /> 2. Submitted copy of the infection prevention control plan contained procedures for sterilization and autoclave maintenance <br /> while the PMU operation, according to the practitioner, will not be conducting any sterilization of any tools associated <br /> with PMU. All tools and instruments used for PMU will be purchased pre-sterilized. <br /> 3. Disinfectant products were not properly listed within the Infection Prevention and Control Plan. <br /> 4. There is no signature at the end of the Infection Prevention and Control Plan acknowledging that training on the Infection <br /> Prevention and Control Plan was completed by practitioner. <br /> CORRECTIVE ACTIONS: <br /> 1. Maintain a copy of the Infection Prevention and Control Plan on site. <br /> 2. Remove any procedures associated to sterilization from the Infection Prevention and Control. Sections regarding <br /> sterilization can be answered with "No sterilizing conducted at facility. Instruments purchased pre-sterilized". <br /> 3. Add the products used as disinfectants within the facility onto the Infection Prevention and Control plan. <br /> 4. Sign and date the end of the Infection Prevention and Control Plan to acknowledge review of the plan has been done by <br /> the practitioner. <br /> 5. Submit updated Infection Prevention and Control Plan to Environmental Health Department. <br /> Final Inspection will be scheduled once the corrections have been made. Facility may not currently <br /> open. <br /> Notes: <br /> 1. Consultation report is emailed to huyenstryker(@yahoo.com <br /> 2. Body art facility is a room located within a nail salon business. <br /> 3. No waste hauler contract was observed on site. Medical Waste Hauler listed on the Infection Prevention and Control <br /> Plan is MedWaste Management <br /> 4. Facility included a biohazard container for biohazardous waste. It was explained to the practitioner that biohazardous <br /> waste containers were not a requirement for body art businesses. The practitioner removed the container from the <br /> procedure area. <br /> 5. All sinks within the facility were properly supplied with pump style soap dispensers, touchless paper towel dispensers, <br /> and hot water. <br /> 6. Operator noted that the walls of the procedure area are not allowed to reach to the ceiling per the fire department. <br /> Facility still needs to adhere to the safe body art regulation in being completely separate from nail activities. Facilties that <br /> don't have floor to ceiling partitions that separates nail activities from body art operations will not be approved. <br /> 7. Operator noted that waxing is the only other service that will be performed within the procedure room. It was noted to <br /> operator that the waxing and the permanent make up operations should only be done on alternating days. One day <br /> dedicated to waxing and the other day dedicated to permanent make up. <br /> 8. Blood borne pathogen training certificate was observed on site. <br /> 9. Noted to operator that lot numbers and ID scans must be recorded for each client to keep track needles used and <br /> provide proof of age for client. <br /> Reinspection on/about: A reinspection fee of$179 per hour may be charged.
