A copy of the disposal manifest or bill of lading will be included in RDM's well <br />destruction report. <br />Construction debris (asphalt, concrete and well boxes) generated during the well destructions will be <br />hauled to a local recycling facility or landfill for disposal. <br />Reporting <br />Following completion of the work, RDM will submit a results report to the CVRWQCB to document that <br />well destructions and disposal of any investigation -derived waste were completed so that the site can be <br />granted "final" closure. In addition, DWR Well Completion Reports (DWR Form 188) will be submitted <br />to the DWR and SJCEHD for each destroyed well. <br />Schedule <br />The proposed scope of work will be initiated within 30 days of receiving the CVRWQCB's written <br />approval of this work plan and the SJCEHD's issuance of the drilling permits. RDM will submit a report <br />summarizing the well destruction and site restoration activities to the CVRWQCB and the SJCEHD (if <br />requested), and RDM or the driller will prepare and submit the DWR well completion reports to the <br />SJCEHD and the DWR within 45 days following completion of the work. <br />Remarks/Signatures <br />The interpretations contained in this document represent our professional opinions, and are based in part, <br />on information supplied by the client.
