This well is planned to be drilled and completed <br /> next to the southside gutter within Murray Street about 200 feet west of Cherokee Lane (Figure <br /> 1). This well will be within the City of Lodi Murray Street. We have submitted an application to <br /> the City of Lodi for an Encroachment Permit. <br /> This well will be a part of an ongoing environmental investigation for soil and groundwater <br /> contamination at the 521 North Cherokee Lane site. The California Regional Water Quality <br /> Control Board(RWQCB), Rancho Cordova office is the agency that is directing our work. Our <br /> contact person at the RWQCB is Michael DeSmet(916) 464-4830. <br /> The Parmar Texaco site has nine existing groundwater monitoring wells (PT-MW 1 to 9)that <br /> exist and numerous soil boring and CPT locations from past work. This site has been active <br /> since 1988. <br /> This package being submitted to SJCEHD for a groundwater monitoring well permit contains the <br /> following: (1)completed SJCEHD monitoring well permit application; (2) copies of RWQCB <br /> directive letters to do the work; (3) front end of the workplan that we developed with background <br /> information. The workplan has been reviewed, approved and directed to be implemented by the <br /> RWQCB (4) a map of the proposed new monitoring well. <br /> The workplan approved by the RWQCB contains an existing Health and Safety Program(HASP) <br /> that meets both federal and California state health and safety guidelines for workers at UST <br /> petroleum hydrocarbon investigations.
