The following is an itemized list of CP22 - HMBP violations that have not been <br /> addressed for EDDIES PIZZA CAFE as of July 07, 2025. <br /> Open violations from May 29, 2025 inspection <br /> Violation#4-1010008-Business plan reviewed and electronically certified as complete/accurate on or <br /> before the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California Environmental <br /> Reporting System (CERS)annually by the due date,January 15. <br /> -The annual submission for 2025 was made late on 4/6/2025, 81 days late. <br /> -The annual submission for 2023 was made late on 1/22/2025, 7 days late. <br /> REGULATION GUIDANCE:The hazardous materials inventory shall be submitted by January 15 of each calendar year <br /> and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9-1010003-Business Owner/Operator ID and Business Activities pages electronically submitted <br /> and are accurate. <br /> OBSERVATION: The Business Owner/Operator Identification form was not submitted or complete. <br /> -Manager Daphney Luna stated that the Secondary Emergency Contact General Manager Sandra Mendez, is no <br /> longer an employee.
