REGULATION GUIDANCE: (e) Owners and operators must pay all applicable local government fees and state surcharge fees. CORRECTIVE ACTION: If Owner/Operator has not obtained a valid permit to operate, or if the permit to operate has expired, a complete application to obtain a current permit to operate must be submitted to the UPA for approval. If facility is not in compliance with one or more requirements of the permit to operate, owner/operator must submit documentation of compliance. All unpaid permit fees must be paid. All applicable violations must be corrected within 30 days. This is a Repeat(Class 1)Violation. 2 23 CCR 16 2613(a)(1)-(4) and (6), and (b), 2690; HSC 6.7 25286 2011009-Complete and accurate application for a permit to operate a UST has been submitted and maintained OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and/or accurate application for a permit to operate a UST or failed submit application for renewal permit. -The facility has an closed California Department of Tax and Fee Administration (CDTFA) Underground Storage Tank account. This account has been closed since 8/10/2023. REGULATION GUIDANCE: (a)An application for an operating permit must be submitted to the Unified Program Agency by the owner or operator. The application must include the following information, to the extent such information is known to the applicant: (2) Facility Information—The name, California Department of Tax and Fee Administration number, location, mailing address, and telephone number of the facility where the underground storage tank is located, and type of business involved, if any. CORRECTIVE ACTION: Submit and/or maintain a complete and/or accurate application for a permit to operate a UST, including for permit renewal. This is a Repeat(Class 2)Violation. 3 23 CCR 16 2613(a)(3) and (b), 2650(a), (b), and (e) 2011011 - Facility has submitted and maintains a complete and accurate UST Monitoring Plan OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and accurate UST Monitoring Plan. -The regular unleaded Mechanical Line Leak Detector(MLLD) model is listed as 99 LD200. The current MLLD is 99 LD-2000. REGULATION GUIDANCE: (c) Monitoring Site Plan —Owners or operators must submit a monitoring site plan for approval by the Unified Program Agency. The monitoring site plan must include, but is not limited to, the following: (1)A scaled diagram indicating the layout of the tank(s) and piping to the extent known, including containment sumps; (2) Locations of all release detection equipment; and (3) If applicable, each vacuum, pressure, or hydrostatic interstitial monitoring zone. CORRECTIVE ACTION: Submit a complete and accurate UST Monitoring Plan and maintain copy onsite for review. This is a Repeat(Class 2)Violation.
