SAN JOAQUIN Environmental Health Department <br /> —COUNTY <br /> CP26 - UST FULL Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STOP N SHOP 11856 W COUNTRY CLUB BLVD, STOCKTON June 12, 2026 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 1 23 CCR 16 2691(b)-(f); HSC 6.7 25284 and 25287 2011010 - Facility obtained valid Permit to Operate from the UPA, <br /> complies with requirements and has paid fees <br /> OBSERVATION: Owner/Operator failed to obtain and/or retain a valid permit to operate from the UPA, and/or failed to <br /> comply with one or more permit requirements, and/or failed to pay permit fees. <br /> -The 2026 UST permit fees have not been paid. <br /> -The monitoring plan was not available for review. <br /> -The monitoring site plan (UST map)was not available for review. <br /> -The UST response plan was not available for review. <br /> REGULATION GUIDANCE: (e) Owners and operators must pay all applicable local government fees and state surcharge <br /> fees. <br /> CORRECTIVE ACTION: If Owner/Operator has not obtained a valid permit to operate, or if the permit to operate has <br /> expired, a complete application to obtain a current permit to operate must be submitted to the UPA for approval. If facility <br /> is not in compliance with one or more requirements of the permit to operate, owner/operator must submit documentation <br /> of compliance. All unpaid permit fees must be paid. All applicable violations must be corrected within 30 days. <br /> This is a Repeat(Class 1)Violation. <br /> 2 23 CCR 16 2613(a)(1)-(4) and (6), and (b), 2690; HSC 6.7 25286 2011009-Complete and accurate application for a <br /> permit to operate a UST has been submitted and maintained <br /> OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and/or accurate application for a permit to <br /> operate a UST or failed submit application for renewal permit. <br /> -The facility has an closed California Department of Tax and Fee Administration (CDTFA) Underground Storage Tank <br /> account. This account has been closed since 8/10/2023. <br /> REGULATION GUIDANCE: (a)An application for an operating permit must be submitted to the Unified Program Agency <br /> by the owner or operator. The application must include the following information, to the extent such information is known <br /> to the applicant: (2) Facility Information—The name, California Department of Tax and Fee Administration number, <br /> location, mailing address, and telephone number of the facility where the underground storage tank is located, and type <br /> of business involved, if any. <br /> CORRECTIVE ACTION: Submit and/or maintain a complete and/or accurate application for a permit to operate a UST, <br /> including for permit renewal. <br /> This is a Repeat(Class 2)Violation. <br /> 3 23 CCR 16 2613(a)(3) and (b), 2650(a), (b), and (e) 2011011 - Facility has submitted and maintains a complete and <br /> accurate UST Monitoring Plan <br /> OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and accurate UST Monitoring Plan. <br /> -The regular unleaded Mechanical Line Leak Detector(MLLD) model is listed as 99 LD200. The current MLLD is 99 <br /> LD-2000. <br /> REGULATION GUIDANCE: (c) Monitoring Site Plan —Owners or operators must submit a monitoring site plan for <br /> approval by the Unified Program Agency.
