Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />March 09,2021 <br />Facility Address: <br />25560 N HWY 99 , Acampo <br />Facility Name: <br />Galt Super Lube <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />RemarksItem # <br />8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br />OBSERVATION: A training program has not been submitted. <br />REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, <br />shall include familiarity with the facility's Emergency response plans and procedures in the event of a release or <br />threatened release of a hazardous material. Training shall cover, but not limited to, all of the following: <br />(A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br />program agency. <br />(B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment. <br />(C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> These training programs may take into consideration of each employee. <br />CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br />and submit to the EHD for approval. <br />This is a Class II violation. <br />10 HSC 25508.1(a)-(f) Failed to electronically update the HMBP within 30 days of substantial change in operations. <br />OBSERVATION: During the inspection it was stated that an ownership change has occured. The previous owner <br />stated the ownership change occurred December 31, 2020; the current owner said he has owned the business <br />since January (2021). The business has not updated and submitted the information within 30 days. <br />REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br />information submitted to the California Environmental Reporting System (CERS): <br /> - A 100 percent or more increase in the quantity of a previously disclosed material. <br /> - Any handling of a previously undisclosed hazardous materials <br /> - A change of business address, business ownership, or business name. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />FA0018142 PR0526785 SC001 03/09/2021 <br />EHD 19-01 Rev. 9/16/2020 Hazardous Materials Program OIRPage 4 of 6 <br />1868 E.
