A dOAQU1N Environmental Health Department <br /> COUNTY <br /> CP22 - HMBP Inspection Report <br /> Facility Name: Facility Address: Date: <br /> BurgerIM 1 4027 E Morada Ln, Stockton CA 95212 March 12 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is (Minor)Violation. <br /> 14 CA Health and Safety Code 25505(a)(4) 1020002-Provided initial and annual training and maintained training records <br /> for a minimum of three years. <br /> OBSERVATION: Employees have never been trained in safety procedures in the event of a release or threatened <br /> release of a hazardous material. <br /> - Rajesh Pathak was unaware that this training was required, he was provided an overview of the type of training that is <br /> required. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment. (C)Evacuation plans and procedures, including immediate <br /> notice,for the business site. This training shall be documented electronically or by hard copy and shall be made <br /> available for a minimum of three years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is(Minor)Violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 11, 2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. Starting July 1, 2024, all in-office CERS help will be provided at the EHD hourly rate ($172).
