OBSERVATION:The site map is incomplete. - In CERS,the site map shows that the street east of the facility is"Name Lane".While on site, it was observed via street signs that the street is instead "Ham"lane. REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map requirements the governing body of the unified program agency finds necessary. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation (Class 2) Violation. 12 CA Health and Safety Code 25505(a)(3) 1010010-Electronically submitted adequate response plan/procedures for release/threatened release of hazmat. OBSERVATION:The emergency response(ER)procedures were inadequate. - In CERS, Section All of the Emergency Response Contingency Plan only indicates Hazardous Materials and does not include Hazardous Waste.This facility has indicated the presence of both Hazardous Materials and Hazardous Waste in the 'Business Activities"section of the CERS Submittal,therefore both boxes should be marked. REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment - evacuation plans and procedures, including immediate notice,for the site CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
