OBSERVATION:The site map is incomplete. There are multiple hazardous materials and hazardous waste storage area within the building and are not identify. In addition to eye wash stations and emergency fog horns that are part of the emergency response equipment are not identify on the site map. REGULATION GUIDANCE: Site map need to contains north orientation, adjacent streets, access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map requirements the governing body of the unified program agency finds necessary. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation (Class 2) Violation. 14 CA Health and Safety Code 25505(a)(4) 1020002-Provided initial and annual training and maintained training records for a minimum of three years. OBSERVATION:The business failed to provide initial and annual training to all employees in safety procedure and failed to document and maintain training records for a minimum of three years. REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment. (C)Evacuation plans and procedures, including immediate notice,for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the training records to the EHD. This is(Minor)Violation.
