Ms. Lucas <br /> called Mara Bernardo (owner)to notify her of the complaint. Ms. Bernanrdo gave permission to view the location of the <br /> sharps waste containers. A wall mounted sharps container and two sharps containers observed in the clinic, in one of the <br /> rooms. The Medical Waste Management packet was provided to Ms. Lucas. <br /> On September 17, 2025, a Certification Statement and waste hauler information was provided to the EHD, via email. <br /> The facility is utilizing Med Pro Waste Disposal for sharps waste disposal. <br /> CORRECTIVE ACTIONS: <br /> Violations corrected.
