Alarm sounded off in one area only. (Is this set for specific areas or should alarm sound <br /> in whole warehouse)? <br /> 2. Windsock was blowing East upon evacuation, wind was strong and direction would <br /> change constantly. <br /> 3.(2) Associates did not hear the alarm; <br /> a. Production associate was in the Filters area and did not hear the alarm. No other <br /> associates made him aware of drill. Assistant Prod. Mgr, Jose went back in <br /> building to look for associate. <br /> b. Groundskeeper forklift operator did not hear alarm, need to create a process <br /> to notifiy associates assigned to outside work. <br /> 4. Some associates were using non-emergency doors vs emergency exits; were not <br /> following Leads who were directing associates out to path to exits. <br /> 5. Bullhorn's were not used during drill - feedback provided - need training how to use <br /> and phrase to use when alerting associates. <br /> 6. Warehouse restrooms and lunchroom cleared, shipping restroom was not checked for <br /> associates/truck drivers. <br /> 7. Visitor log was not checked to verify contractors on-site. <br /> 8. Lead had daily manifest and stated he checked all associates assigned to his line, he <br /> did not check off name and stated he did a visual check in. <br /> 9. Bagger operators hesitated to evacuate; instead trying to turn off machines. <br /> 10. Should there be a portal First Aid Kit handy to take outside if needed? <br /> 11. Training of evacuation drill and review newly updated MAPS. <br /> 12. Outside contractor was alerted; he proceeded to stick around and chat vs evacuating. <br /> 13: Start time of drill: 1 :04pm, End time of drill: 1:10pm, all associates accounted for at <br /> end time of drill. <br /> 14. Weather was 102 degrees. <br />
