Keep copy on site at all times. <br />Okay to operate. Obtain permit ASAP. <br />PE1612 $364 permit fee. <br />5021 form to be updated. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
