RACKS ARE NEVER SWITCHED OUT. COCKROACHES ARE <br /> RANDOM THROUGHOUT THE FACILITY(DINING ROOM, KITCHEN, INSIDE REFRIGERATOR, BACK ROOM).THERE ARE <br /> ALSO FLIES INSIDE THE FACILITY EVERYWHERE <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> COMMENTS AND OBSERVATIONS <br /> Follow up inspection. Arrived on site and met with staff. Walk in cooler has been cleaned. Walls, racks,floors, and fan covers are <br /> clean.
