A dOAQU1N Environmental Health Department <br /> COUNTY <br /> CP22 - HMBP Inspection Report <br /> Facility Name: Facility Address: Fa <br /> JACK IN THE BOX#534 2592 W MARCH LN, STOCKTON20 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation (Class 2) Violation. <br /> 12 CA Health and Safety Code 25505(a)(3) 1010010-Electronically submitted adequate response plan/procedures for <br /> release/threatened release of hazmat. <br /> OBSERVATION:The emergency response(ER)procedures were lacking or inadequate. <br /> -E3 (Emergency evacuation assembly area)and E4 (emergency evacuation routes)are blank on the Emergency <br /> Response/Contingency Plan submitted <br /> -The Emergency Response/Contingency Plan submitted lists emergency response equipment(first aid kit, eyewash <br /> station)that were not observed on site during the inspection. <br /> REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a release <br /> or threatened release of hazardous materials, including, but not limited to, the following: <br /> -Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -Evacuation plans and procedures, including immediate notice,for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation (Class 2) Violation. <br /> 14 CA Health and Safety Code 25505(a)(4) 1020002-Provided initial and annual training and maintained training records <br /> for a minimum of three years. <br /> OBSERVATION:The facility could not provide training records for the last three years during the inspection. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br /> agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C)Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> FA0001999 PR0520651 SCO01 05/20/2025 <br /> EHD Rev.12/06/2021 Page 3 of 4 CP22-HMBP OIR <br /> 1868 E.
