I <br /> was only able to close one violation between the three inspections, all other violations remain open. <br /> Please see the feedback below- <br /> HMBP: <br /> • Violation #3—Open. Nothing was submitted to demonstrate that emergency <br /> response/contingency plan is now maintained on site. <br /> • Violation #10—Open. None of the observations noted in the inspection report have been <br /> corrected in CERS. <br /> • Violation #11 —Open.The site map has not been updated to correct the observations listed in the <br /> violation. <br /> • Violation #14—Open.The training records were sent for April 2023 and April 2024. The training <br /> records observed on site were for November 2025. Employees were overdue for training from April <br /> 2025 to November 2025. Provide a statement indicating how the facility will ensure this will not be <br /> an issue going forward. <br /> Hazardous Waste: <br /> • Violation #5—Open.Training records alone are not sufficient to close this violation. Please see the <br /> corrective action guidance in the report. <br /> • Violation #7—Open. This was not addressed. <br /> • Violation #14—Open. None of the items listed in this violation have been addressed. Complete <br /> hazardous waste determinations must be made for each waste stream listed and supporting <br /> documentation must be submitted. <br /> • Violation #18—Open.This has not been addressed. <br /> 1 <br />
