A dOAQU1N Environmental Health Department <br /> COUNTY <br /> CP22 - HMBP Inspection Report <br /> Facility Name: Facility Address: TDate: <br /> SHOP N GO 3 4511 PACIFIC AVE, STOCKTON ebrua 05 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, shall <br /> include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened <br /> release of a hazardous material. Training shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br /> agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C)Evacuation plans and procedures, including immediate notice,for the business site. <br /> These training programs may take into consideration the position of each employee. <br /> CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and <br /> submit to the EHD for approval. <br /> This is (Minor)Violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on 2/11/2025 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 3/13/2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. <br /> Starting July 1, 2024, all in-office CERS help will be provided at the EHD hourly rate ($172).
