Geosyntec O <br /> consultants <br /> • Geosyntec will obtain all necessary access agreements from property <br /> owners/tenants as needed; and <br /> • The Health and Safety Plan(HASP)will be updated and available on-Site during <br /> all field activities. A copy of the HASP is included in Appendix A. Prior to the <br /> start of field activities, a safety tailgate meeting will be conducted. <br /> 5.2 Destruction of Wells <br /> The well destructions will be conducted by a C-57 licensed drilling contractor working <br /> under the supervision of a California licensed professional geologist. <br /> The screened interval and filter pack of each well to be destroyed will be removed via <br /> shot perforation and the boring backfilled with Portland cement grout to 5 feet below <br /> ground surface (bgs) under inspection by SJCEHD. The well box will be removed and <br /> the upper 5 feet will be over drilled using the hollow stem auger(HSA) drilling method' <br /> with an auger diameter equal to or greater than the original well borehole. In accordance <br /> with the California Well Standards, the top 5 feet bgs will be backfilled with native soil <br /> (California Department of Water Resources [DWR], 1991). The surface will be patched <br /> to match the surrounding area(i.e., soil, concrete, or hot patch asphalt). <br /> Alternatively, if destruction via shot perforation is not possible for any reason (i.e., <br /> contractor availability, scheduling, use restriction), well materials may be removed by <br /> over drilling the well to total depth via HSA with an auger diameter equal to or greater <br /> than the original well borehole. Once the well materials have been removed,the borehole <br /> will be tremie grouted with Portland cement grout from the bottom of the borehole to 5 <br /> feet bgs under inspection by SJCEHD. The top 5 feet will be backfilled with native soil <br /> and the surface patched to match the surrounding area. <br /> Vadose zone wells will be destroyed by pressure grouting, over drilling the upper 5 feet, <br /> backfilling with native soil, and patching the surface to match the surrounding area. <br /> The soil cuttings and well casing removed from the ground as part of the well destruction <br /> activities will be temporarily stored in drums or bins, the soil cuttings will be sampled <br /> CVRWQCB will be notified if an alternative drilling method will be used. <br /> Well Destruction Work Plan 5 June 27,2023 <br />
