(c) of Waste Discharge Requirements R5-2015-0094 (WDRs).The report
includes a condition survey of all monitoring wells identified in Finding 43; no new monitoring
wells have been installed since the date of the WDRs.The survey identifies wells that could not be
recovered in the field and those that have been damaged. For damaged wells, the workplan
describes proposed abandonment procedures that comply with California Well Standards Bulletin
74-90 (June 1991); State of California Bulletin 94-81 (December 1981); and any more stringent
standards adopted by the state or county pursuant to Water Code section 13801.All wells
identified in Finding 43 that were previously abandoned are also included in the well inventory
table provided in the report. For each monitoring well that will not be destroyed, the report
provides a specific plan for well head improvements, if needed,to protect the well from future
damage. The report also includes an inspection and maintenance plan to ensure that wells are
properly maintained for continued use. Lastly, as required by the WDRs this Groundwater
Monitoring Well Condition Survey Report and Destruction Plan includes ownership information for
existing wells and describes the City's right of access.
If you have any questions regarding this document, please contact Christina Lucero at 530-759-
2484 (cluceroPhydrofocus.com).
Sincerely,
C'
No. p(;9202
l •
'6r
Christina Lucero, P.G. -7 Wl John Fio
Hydrologist
'% " Principal Hydrologist
pF C, /.,,: i
P.O.
