CVICH Lathrop 1,LW 555 Capitol Mall,NId*Floor Sacramento,CA 95U4 PERMIT TO ENTER CONSENT FOR ACCESS TO MONITORING WELL ON PROPERTY CVICH Lathrop 1,LLC provides permission to the City of Lathrop,its employees,agents,contractors,and other authorized representatives to enter in,over,and upon my property identified below for the sole purpose of conducting monitoring well inspections and surveys,groundwater monitoring,and any well maintenance as necessary with respect to the two(2)existing monitoring wells currently located on the property,all on the terms and conditions set forth herein. Upon request to the City,water quality information collected from the well shall be shared. The City of Lathrop and its representatives will give reasonable notice before conducting any activities on the property as to minimize disruptions or inconvenience to the owner's operation. The rights and privileges hereby granted to the City may,at the option of the City,be exercised by any authorized agent or contractor. It is understood and agreed that the City or its agents will leave the property in a clean and orderly condition,and will repair,replace,or compensate for any existing improvements that are damaged during the course of work. Project team members and the project consultant have been thoroughly instructed in proper field procedures and conduct. The following rules will be followed: • Property owners will be given at least 24 hours'notice by telephone or mail prior to entry. Please note that your telephone number is requested on page 2 of this Permit. • Gates will be left as they were found. • No pets or firearms will be permitted. • Smoking will not permitted on the property. • No trash or other evidence of field visits will be left on the property. • All field staff will carry identification. • Vehicle speeds will be kept to five(5) miles per hour on unpaved roads in agricultural properties to minimize dust. Owner is welcome to accompany or observe any of the crews working on their property at any time. The City agrees to indemnify,defend,and hold the undersigned parties harmless from and against any and all loss,liability,expense,claims,costs,suits,damages,and attorney's fees,arising directly out of or City of Lathrop,Permit to Enter Page 1
