January 25, 2016 Well Head Protection Improvements Plan <br /> 2.1. Protective Bollards <br /> 28 wells require the installation of protective bollards to ensure future safety of the well.At each <br /> well site,three to four bollards should be installed around the well by digging an approximately 2- <br /> foot deep hole and setting the bollard with concrete.The distance between bollards should be <br /> chosen to ensure no vehicular damage can occur to the monument or well casing. <br /> 2.2. Monument Lids <br /> The standing monument lid at MWR-4 is no longer attached to the standing monument and the <br /> standing monument lid is completely missing at MWR-7.At MWR-4,the hinged side of the existing <br /> lid should be re-welded to the monument base to ensure the well is tamper-proof. Similarly at <br /> MWR-7,a new monument lid should be purchased and the hinged side should be welded to the <br /> monument base. <br /> 2.3. Rodent Holes <br /> At NMW-2 and MW-S2,rodent holes were found at the base of the well.These holes do not appear <br /> to affect the annular seal. However to ensure the annular seal is properly maintained,these rodent <br /> holes should be filled with neat cement. <br /> 2.4. Well Casing Height <br /> During well reconnaissance, HydroFocus, Inc. personnel noticed the top of well casing MWM-6 was <br /> damaged and missing the measuring point. In order to properly secure the well plug,the well casing <br /> was cut down slightly.The measuring point elevation on this well should be re-marked and re- <br /> surveyed. <br /> At MWM-27,the top of the well casing exceeds the height of the flush mount vault, preventing the <br /> vault lid from properly securing. Ideally,the flush mount vault should be moved upwards to <br /> completely enclose the well casing,preserving the existing measuring point. Due to unforeseen <br /> constraints,this option may not be a viable,in which case the well casing should be cut down to fit <br /> within the flush mount vault and the measuring point should be re-marked and re-surveyed. <br /> HydroFocus, Inc. 3 <br />
