This type of work would require <br /> San Joaquin County EHD Permit Type 5. UST Retrofit- No Fee Permit. A permit was not obtained. <br /> REGULATION GUIDANCE: § 2691(d)A paper or electronic copy of the permit and all conditions and attachments, <br /> including the monitoring program, must be readily accessible at the facility. <br /> Per UST Permit Conditions: 11) Construction repair and/or removal permits are required from EHD prior to any change, <br /> repair or removal of UST system equipment <br /> CORRECTIVE ACTION: A permit for the work conducted must be pulled. A Failure to Obtain a Permit Penalty will apply <br /> See Underground Storage Tank (UST) Permit Guidance document for details. <br /> This is a Repeat(Class 2)Violation. <br /> 3 23 CCR 16 2613(a)(3) and (b), 2650(a), (b), and (e) 2011011 - Facility has submitted and maintains a complete and <br /> accurate UST Monitoring Plan <br /> OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and accurate UST Monitoring Plan. The <br /> monitoring plan for both tanks list that the pipnig secondary is vacuum monitored but the sensors listed for piping is <br /> 208/304. 304 is a sump interstitial sensor. <br /> REGULATION GUIDANCE: §2650(b)(2)The release detection methods and release detection equipment, identified by <br /> name and model, to be used for performing the monitoring; <br /> CORRECTIVE ACTION: This was corrected on site. The montoring plan was corrected to list the sensors as 208/001 for <br /> sump sensor and vacuum sensor in CERS during the inspection. <br /> This is a Repeat(Class 2)Violation. <br /> 4 23 CCR 16 2613(a)(4) and (b), 2650(a), (c), and (e) 2011012 - Facility has submitted and maintains a monitoring site <br /> plan <br /> OBSERVATION: Owner/Operator failed to submit and maintain a monitoring site plan for approval by the UPA. The site <br /> map has the following missing/inaccurate: <br /> 1) hydrostatic monitoring zone not listed <br /> 2)vacuum zones not listed <br /> 3) piping layouts incorrect <br /> REGULATIONS GUIDANCE: § 2650(c) Monitoring Site Plan —Owners or operators must submit a monitoring site plan <br /> for approval by the Unified Program Agency.
