The following is an itemized list of CP22 - HW LQG violations that have not been <br />addressed for FLAG CITY TRUCK SERVICE as of April 21, 2026. <br />Open violations from June 19, 2025 inspection <br />Violation #5 - 3020002 - Facility personnel are trained annually and maintain required documentation until <br />closure. <br />OBSERVATION: The business operator could not produce documentation during the inspection that demonstrates <br />employees have complete hazardous waste training as specified in this section. Required training documents were <br />also not provided during the inspection. <br />REGULATION GUIDANCE: New employees shall be trained within 180 days of hire and annually thereafter as <br />specified in Title 22 California Code of Regulations section 66265.16. The owner or operator shall maintain training <br />records on current personnel until closure of the facility and training records on former employees for at least three <br />years from the date the employee last worked at the facility. <br />CORRECTIVE ACTION: Immediately train all employees who work with hazardous waste in compliance within the <br />time frames and training materials required by this section. Provide training documentation to the San Joaquin County <br />Environmental Health Department (EHD) within 30 days. The documentation must include a list of training topics, date <br />of training, job title, name of employee, written job description (qualifications and duties), and employee signed (or <br />certified) record that documents the training. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #9 - 3010010 - Signed copy of Uniform Hazardous Waste Manifest kept for 3 years. <br />OBSERVATION: The business operator could not provide hazardous waste disposal records for the past three years <br />during the inspection. <br />The following Uniform Hazardous Waste Manifests were missing: <br />- Manifest Number 030062818WAS with shipment date 12/20/2022 <br />- Manifest Number 030065341WAS with shipment date 7/18/2023 <br />- Manifest Number 030066931WAS with shipment date 11/7/2022 <br />- Manifest Number 030070475WAS with shipment date 8/23/2023 <br />REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br />with section 66262.23(a) for three years or until the generator receives a signed copy from the designated facility <br />which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br />waste was accepted by the initial transporter. <br />CORRECTIVE ACTION: Provide a copy of disposal records for the missing manifests listed above. Provide these <br />records and a corrective action statement to the EHD within 30 days. <br /> <br />Page 1 of 7
