A copy was obtained from the EHD Public Records. <br /> This is a (Minor)Violation. <br /> 99 23 CCR 16 2613(a)(8) and (b), 2630(b) 2011004 - Owner/Operator submitted DO form within 30 days of UST install or <br /> DO change (TCR 10) <br /> OBSERVATION: Owner/Operator did not submit the Designated Underground Storage Tank Operator Identification Form <br /> to the UPA within 30 days installing UST or changing the DO. <br /> -A new designated operator, Kaleb Winkler, began conducting inspections on 2/6/26 and notification was not provided to <br /> the EHD within 30 days of the change. <br /> REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator <br /> Identification Form" no later than 30 days after any change in the designated UST operator(s). <br /> CORRECTIVE ACTION: This was corrected on site. The updated form was submitted to CERS. <br /> This is a (Class 2)Violation. <br /> Overall Inspection Comments <br /> All observed violations were corrected on site. No follow up is needed. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. Starting July 1, 2025, all in-office CERS help will be provided at EHD hourly rate ($179).
