Docusign Envelope ID: F84E4B8A-ACA5-8A76-821A-24682E9E8ECC <br /> Statement Regarding Missing UST Installation and Testing Records <br /> In response to the inspection observation for the facility located at 642 N Hunter St, I have <br /> conducted a thorough and diligent search for all available records relating to the underground <br /> storage tank (UST) system, including installation, repairs, linings, integrity certifications, tank <br /> calibration charts, and unauthorized release reports. <br /> Despite these efforts, I have not been able to locate the original records requested, including but not <br /> limited to: installation test results, manufacturer checklists and manuals, and as-built drawings. <br /> To the extent possible, I will obtain and utilize any available documentation from the Environmental <br /> Health Department's public records to reconstruct the installation and testing history of the UST <br /> system.
