These requirements cannot be modified. <br /> 1. Submit Water Provision Declaration form to the Environmental Health Department for review. <br /> 2. Submit a Small Public Water System preliminary technical report to the California State <br /> Water Resources Control Board, Division of Drinking Water (Water Board) prior to issuance <br /> of building permits and at least six months before initiating construction of any water related <br /> improvement, as defined. The issuance of a permit to operate a small public water system <br /> by the local primacy agency [Environmental Health Department (EHD)] is prohibited without <br /> the concurrence of the Water Board. If the Water Board determines that an onsite well shall be used as the potable water <br /> source, a permit application to operate Small Public Water System shall be submitted to the <br /> EHD for approval prior to issuance of building permits. To issue a permit to operate, <br /> concurrence from the Water Board is required. A yearly permit to operate a public water <br /> system will be required by the Environmental Health Department prior to sign off of the <br /> certificate of final occupancy (San Joaquin County Development Title, Section 9-602.010 <br /> and 9-601.030). <br /> 4. The supplier must possess adequate financial, managerial, and technical capability to <br /> assure delivery of pure, wholesome, and potable drinking water in accordance with San <br /> Joaquin County Development Title, Sections 9-602.010 and 9-601.030 and C.C.R., Title <br /> 22, and Health and Safety Code, Section 116525 116570. <br /> 5. Submit two (2) hardcopy sets, or one (1) electronic version, of food facility plans to the <br /> Environmental Health Department for review and approval prior to issuance of building <br /> permit(s) (California Retail Food Code, Article 1, 114380). The fee will be based on the <br /> current schedule at the time of payment. <br /> 6. A valid permit from EHD is required prior to operating food facility (California Retail Food <br /> Code, Chapter 13, Article 1, Section 14381). <br /> 1868 E.
