GROUND ZERO ANALYSIS,INC. Quality Control/Quality Assurance Plan <br /> Site drawings, maps or air photos will be used, as appropriate, for establishing and recording the <br /> location of sample collection, or making notations for field observations. <br /> Photographs will be taken at all areas of interest and sample locations. Photos will also be taken <br /> to supplement field notes, record sampling activities or to note unusual conditions. For each <br /> photograph taken,the following information will be written in the logbook or recorded in the field <br /> photography log: <br /> • Time, date, location, and weather conditions <br /> • Description of the subject photographed <br /> • Name of person taking the photograph <br /> 1.2 Sample Identification <br /> Samples will be uniquely designated with a sample identification number conforming to the <br /> requirements of RP-006 Sample Collection Procedure,DMS-7209030-RIP-45 Rev. 0,April 2015c. <br /> The following information shall be listed on each sample containers: <br /> • Date and time sample was obtained <br /> • Sampler's initials <br /> The information will be marked with an indelible marker on a sample label or a tag affixed to each <br /> sample container. A Sample Tracking Log will be maintained by the sampling personnel to track <br /> all collected samples. <br /> 1.3 Location of Samples <br /> The sampling locations will be accurately located by using measured distances from permanent <br /> landmarks on the scaled site maps,by survey or by GPS instrumentation.The method and accuracy <br /> will be determined by project Data Quality Objectives (DQOs). <br /> 1.4 Sample Custody <br /> Sampling personnel will complete the COC form prior to transferring samples to the laboratory <br /> either by direct delivery or courier service. A sample is under custody, if one or more of the <br /> following criteria are met: <br /> • It is in the sampler's possession <br /> • It is in the sampler's view after being in possession <br /> • It is in a designated secure area <br /> At a minimum, for each sample, the required analyses, sample collection date/time, and the <br /> individuals or organizations performing the sample collection, shipment, and receipt will be listed <br /> on the COC.
