PROPOSED SCOPE OF WORK This section describes the proposed approach and work elements for well destruction. 3.1 Pre-Field Activities Prior to initiating fieldwork at the Site, the following pre-field activities will be completed. 3.1.1 Health and Safety Planning Current field conditions will be reviewed by staff before commencing field activities. The assessment will include the following items: • Evaluating known existing utilities in relation to the proposed boring locations • Ensuring safe access and movement within the work area • Assessing overall health and safety conditions for the planned well destructions After a review of field conditions has been completed, a Site-specific task hazard analysis (THA) will be prepared to support safe work practices. The THA will contain procedures for hazard identification and mitigation, stop work authority, emergency response (including a map of the nearest hospital and emergency contact information), incident reporting, and use of appropriate personal protective equipment. Hazards identified and controls developed will be communicated with all field personnel prior to executing the proposed scope of work. Prior to the start of field activities each day, a safety tailgate meeting will be conducted between Geosyntec field staff and subcontractors to discuss the field activities to be performed that day and the THA elements listed above. 3.1.2 Notifications,Access, and Permitting Prior to the start of field work, the following will be performed: • Coordinate with the property owner of the agricultural parcels for access to MW-9 through MW-15 • Coordinate with UPRR,CFNR, and National Railroad Safety Services(the UPRR-and CFNR-designated railroad flagging service) for access to MW-5 • Select and retain utility locating and drilling subcontractors • Create a ticket with Underground Service Alert of Northern California (USA-North) • Obtain a drilling permit from the San Joaquin County Environmental Health Department(SJCEHD) The CVRWQCB will be notified of the planned work schedule a minimum of two weeks prior to the start of field activities.
