Geosyntec <br /> consultants <br /> Geosyntec HSE Procedures referenced herein are available on Geosyntec's HSE SharePoint site TASK HAZARD ANALYSIS <br /> and should be consulted,as appropriate,to ensure requirements are met.This THA has been (Ver.4.0,9-8.2023) <br /> prepared per"HSE STD-105,Project Safety Management Handbook" <br /> Part A — PROJECT/TASK INFORMATION <br /> Project/Site Name: Olin ChlorAlkali Facility F Project Number/Org:F WR3313B <br /> Site Address: 26700 South Banta Road,Tracy,CA <br /> Task&Worksite Geosyntec will oversee the decommissioning of eight 2-in diameter groundwater monitoring wells(MW-5 and MW-9 to <br /> Description: MW-15 in Work Plan)ranging from 27 to 78 feet deep by pressure grouting in accordance with SJCEHD specifications. <br /> Work will be conducted by a C-57 licensed drilling subcontractor. <br /> • Neat Portland cement will be pumped through a tremie pipe from the total depth of the well to approximately <br /> two feet bgs.The well will then be pressurized to 25 psi and maintained for 5 minutes to force the sealing <br /> material through the existing perforations of the well casing into the surrounding formation.The above-grade <br /> well monument and any surrounding well box/collar/pad will be removed,and a mushroom cap will be poured <br /> over the pre-existing well area until flush with the surrounding grade. <br /> • MW-5 is located within Union Pacific Railroad right of way.An access agreement has been obtained and will <br /> be endorsed by the C-57 driller. Flagging service by National Railroad Security Service(NRSS)will be <br /> coordinated to watch out for and direct train traffic during well destruction.Geosyntec staff will receive Union <br /> Pacific Roadway Worker Protection training before start of the work.Based on previous utility locating at the <br /> nearby MW-6,a subterranean Kinder Morgan natural gas pipeline runs parallel to the train track within 10 feet <br /> of MW-5.Therefore, no drilling or mechanical methods will be used during well destruction activities. Kinder <br /> Morgan will be notified in advance,and a representative might be present to observe well destruction.
