Wong,Jeff
From: Marc Olds <Marc@hydrofocus.com>
Sent: Tuesday, February 25, 2020 3:56 PM
To: Wong,Jeff
Cc: Steve Deverel
Subject: HydroFocus Monitoring Well Application
Attachments: Work Plan - Staten Monitoring Wells.pdf;well_application_signed.pdf;
master_site_record_signed.pdf; pg_license_cards.pdf
Hi Jeff,
This is Marc Olds from HydroFocus. We spoke on the phone a few weeks ago about permitting for monitoring wells on
Staten Island in the delta.
Our application materials are attached to this email. Please look over these documents to make sure everything is OK to
proceed. We will submit payment once we confirm that our documents are in order.
A few notes:
We have decided to install 2 wells for now instead of the 4 we talked about previously.
I've also attached a scan of our principal's Professional Geologist card. Will this be sufficient? We can provide more
formal documentation if necessary. As we discussed on the phone, these wells will be monitoring wells dug to 6ft, so the
work will be overseen by our registered geologist and we will not have a C-57 driller.
The work plan includes some details for our well construction including a construction diagram. There is also a map of
the well locations on Staten Island. The area is entirely farmland and there are no sewage or utility lines near the sites. I
can provide the actual coordinates for each well site if you'd like those as well. The work plan also includes an excerpt
from our monitoring plan for this project. I only included the section relevant to the monitoring wells, but I can provide
the full 68-page plan if you so desire. Finally, our Health and Safety plan is appended to the end.
Feel free to reach out to us if you have any questions/concerns.
Office: (530) 759-2484
My Cell: (209) 777-1413
Thank you
Marc Olds
Hydrologic Technician
HydroFocus, Inc.
marc@hydrofocus.com
Office:(530)759-2484 Ext: 105
Cell:(209)777-1413
1
