A hazardous <br />waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without obtaining <br />and maintaining an Identification Number. To maintain an active Identification Number you must annually complete the <br />Electronic Verification Questionnaire (eVQ). The facility eVQ expired on 12/31/2024. <br />CORRECTIVE ACTION: Submit documentation to the CUPA demonstrating that you have obtained an Identification <br />Number, or taken necessary steps to reactivate an inactive number. Ensure that you complete the annual Electronic <br />Verification Questionnaire to maintain an active Identification Number. <br />This is a (Class 2) Violation. <br />25 22 CCR 12 66262.16(b)(4)(A), 66262.17(a)(5)(A) 3030007-Labeled all containers or portable tanks containing hazardous <br />waste <br />OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. One 55 gallon blue poly <br />drum containing 15 gallons of waste oil did not have a date when hazardous waste was first added. <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the words <br />Hazardous Waste name and address of generator, hazardous properties, physical state, composition (contents), <br />accumulation start date and initial accumulation start date for Very Small Quantity Generators <br />CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. A complete report was <br />finished on July 9, 2026 and replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br />within 30 days (8/9/2026) of receiving the complete inspection report. <br />1868 E.
