The plan was
last reviewed on 01/29/2018 and was due for a 5-year review by 01/29/2023. Review and update the SPCC plan. Once this
has been completed, update the "Date of SPCC Plan Certification or Date of 5-Year Review" in CERS. The SPCC plan does
not need to be uploaded in CERS.
THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
Received by: Inspector Phone:
VICKY VANG-LEE, REHS
Inspector:September 14, 2023 Date:
Printed Name and Title:
Emailed to Patty Olivares, Environmental Services Specialist
(209) 616-3028
FA0019669 PR0540207 SC001 09/14/2023
EHD 28-01 Rev. 01/20/22 Aboveground Petroleum Storage Act OIRPage 1 of 1
1868 E.
