Hazelton Ave. <br /> Stockton, CA 95205 <br /> Subject: Well and Boring Permit Application <br /> Lincoln Village Shopping Center <br /> On behalf of the Lincoln Center Environmental Remediation Trust (LCERT) Geosyntec <br /> Consultants Inc. (Geosyntec) is planning to destroy forty monitoring wells and piezometers at <br /> 818 W Lincoln Road, 6471 Pacific Avenue, in the City of Stockton right-of-way along Pacific <br /> Avenue, and in San Joaquin County right-of-way west of Gettysburg Place. One piezometer <br /> slated for destruction is located at 818 W Lincoln Road, nine piezometers and monitoring wells <br /> are located at 6471 Pacific Avenue, twenty-four piezometers and wells are located in City of <br /> Stockton right-of-way along the Pacific Avenue frontage road, and six wells are located in City <br /> of Stockton right-of-way in residential neighborhoods west of Gettysburg Place. Separate Site <br /> Mitigation Well & Boring Permit Applications have been included for each of the four work <br /> areas. A figure showing the locations of all piezometers and monitoring wells Geosyntec plans to <br /> destroy is included with the Site Mitigation Well & Boring Permit Applications. Encroachment <br /> permits will be obtained from the City of Stockton and San Joaquin County Department of <br /> Public Works prior to implementing the work. Access is currently being requested to destroy the <br /> single piezometer at 818 W Lincoln Road (Lincoln Elementary School). Work will not <br /> commence on the property until access has been granted by the property owner. <br /> The work is being implemented under the oversight and approval of the Central Valley Regional <br /> Water Quality Control Board (CVRWQCB). The selected driller, Penecore Drilling, is a C-57 <br /> certified environmental driller. Please find the enclosed well/boring permit application package. <br /> Included in this application package are the following: <br /> • Attachment 1 — Site Mitigation Well & Boring Permit Applications for borings/wells in <br /> the public right-of-way and each parcel, Declaration Form, and Site Plan Figure that <br /> shows the locations of each well; <br /> • Attachment 2—Well Destruction Work Plan; <br /> • Attachment 3 —CVRWQCB Approval for the Work Plan; <br /> • Attachment 4—Access Agreement(Lincoln Village Shopping Center); and <br /> • Payment check in the amount of$2,106 ($486 for the Work Plan Review fee, $1,620 for <br /> initial well/boring inspection time) <br /> engineers I scientists I innovators <br />
