Other requirements may also <br /> apply. These requirements cannot be modified. <br /> 1. Submit Water Provision Declaration form to the Environmental Health Department for review. <br /> Applicant shall contact Natalia Subbotnikova, Program Coordinator, Small Public Water System <br /> Program, at (209) 468-0338, with any questions. <br /> 2. Applicant shall contact Natalia Subbotnikova, Program Coordinator, Small Public Water System <br /> Program, at (209) 468-0338, to determine if the existing well can be permitted as a public water <br /> system prior to issuance of building permits. If a public water system is required, applicant shall submit <br /> a Small Public Water System preliminary technical report to the California State Water Resources <br /> Control Board, Division of Drinking Water (Water Board) at least six months before initiating <br /> construction of any water related improvement, as defined. The issuance of a permit to operate a <br /> small public water system by the local primacy agency (EHD) is prohibited without the concurrence <br /> of the Water Board. Please contact Gena Farley with the SWRCB Division of Drinking Water at <br /> Gena.Farley@waterboards.ca.gov or 209-948-7488, concerning the requirements for preliminary <br /> technical report submittal prior to issuance of building permits. <br /> If the Water Board determines that an onsite well shall be used as the potable water source, a permit <br /> application to operate Small Public Water System shall be submitted to the EHD for approval prior to <br /> issuance of building permits. To issue a permit to operate, concurrence from the Water Board is <br /> required. A yearly permit to operate a public water system will be required by the EHD prior to sign <br /> off of the certificate of final occupancy (San Joaquin County Development Title, Section 9-602.010 <br /> and 9-601.030.). <br /> The supplier must possess adequate financial, managerial, and technical capability to assure delivery <br /> of pure, wholesome, and potable drinking water in accordance with San Joaquin County Development <br /> Title, Sections 9-602.010 and 9-601.030 and C.C.R., Title 22, and Health and Safety Code, Section <br /> 116525116570. <br /> 3. A soil suitability and nitrate loading study incorporating proposed staff and customer use shall be <br /> submitted to the Environmental Health Department, indicating that the area is suitable for septic <br /> system usage. The studies must be approved by the Environmental Health Department prior to <br /> issuance of building permit(s). (San Joaquin County Development Title, Section 9-1105.2(d)).
