Thomas Baeten has not been with the company <br />since late 2017, but is still identified as the Primary Emergency Contact. This information must be current and <br />submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br />at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />5 HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for the antifreeze, propane, diesel, and urea are not complete/accurate. <br />The CAS No. and hazardous material type for antifreeze has not been identified; the location for the propane, diesel, <br />and urea do not identify the current location north of the shipping doc. This information must be current and <br />submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br />at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />The site map does not show the diesel exhaust fluid, emergency response equipment (spill control equipment in <br />trailer, fire extinguishers, first aid supplies), access and exit points (shop, office, shipping doc), map does not <br />indicate there are no storm drains at the facility, and did not include a legend/key to identify the codes used to <br />denote hazardous material storage containers. Also, the map shows Performance Dr., located adjacent to the facility <br />when it is actually about a half mile north of the site. A site map shall contain a north orientation, loading areas, <br />internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br />staging areas, hazardous material handling and storage areas, and emergency response equipment. Immediately <br />log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or <br />updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures were not complete/accurate. Section C. does not identify the correct phone <br />number for the EHD (209) 468-3420 and Section E. does not indicate the evacuation route map(s) are posted as <br />required.
