LAWDEPARTMENT <br /> UUNITEOSTATES _ <br /> POSTAL SERVICE <br /> October 27,2017 <br /> San Joaquin Environmental Health Department <br /> 1868 E.Hazelton Avenue <br /> Stockton,CA 95205 <br /> RE:Invoice No.IN0297190 <br /> Dear Sir or Madam: <br /> This letter pertains to the Certified Unified Program Agencies (CUPA) billing notice (invoice <br /> no.IN0297190) for the U.S. Postal Service facility at 3131 Arch Airport Rd, Stockton, CA 95213. Please <br /> find enclosed payment of the Generator Fee ($1,949.00), CUPA State Surcharge ($49.00), CERS <br /> Processing Fee ($25.00) and APSA Surcharge ($26:00), totaling $2,049.00.
