Univar USA, Inc. <br />Natural Disaster Checklists <br />Section 6 <br />Exhibit 1 <br />06/14/11 <br />Page 3 of 3 <br />Post — Hurricane & Flood Checklist <br />Date & Time Completed <br />Inspect facility for damage. <br />Document and photograph if available: <br />Electrical; no downed lines, all electrical circuits safe to operate <br />....._._ <br />Structural stability checked; facility walls, walk-ways, etc. <br />Tankfarm integrity; damaged tanks, piping, valves, dikes. <br />Loss of Product; identify any damaged product, quarantine and <br />document for insurance purposes. <br />Visually inspect support columns and cross members for <br />possible movement. <br />Visual inspection of the roof, either external or internal. <br />External inspections can only be conducted by employees that <br />are current on Elevated Work training and only if the building <br />has an affixed OSHA approved ladder (ladder permanently <br />affixed and has a cage). <br />ROM contacted and informed of facility status. <br />RRM contacted and advised if any product loss, environmental <br />releases or off-site consequences. <br />Environmental Contractor contacted for clean-up, if needed. <br />Applicable contractor contacted to conduct building repairs; i.e. roof, <br />asphalt, cement, etc. <br />The potential for employee injury is dramatically increased following a hurricane, flood <br />or severe storm. For this reason it is imperative that no employees be allowed back into <br />the facility until the Emergency Coordinator has completed a facility inspection and has <br />deemed the facility safe.
