San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PAREX INC 11290 S VALLEJO CT, FRENCH CAMP October 26, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> The following chemicals were found inside the production building and have not been reported to CERS chemical <br /> inventory: 305 gallons of Silres BS 4004, 275 gallons of Liprent T, 130 gallons of Mergal 395, 330 gallons of <br /> Alcogum 296, 165 gallons of Maxxon, 275 gallons of ADMIX A48i, 325 gallons of Velsiflex 328, 1,925 gallons of <br /> Acronol 4848, 550 gallons of Rovace 571, 55 gallons of Tafigel , 55 gallons of Methyl Carbitol, and 55 gallons of <br /> Supersperse. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed and submitted with all the required elements. The site map did not indicated which <br /> buildings contain the Hazardous materials/chemicals. Building D is incorrectly marked on the site map as Building <br /> C. Emergency shut offs/E-Stops for the chemical mixers are not indicated on the site map. The Silos(chemical <br /> storage)are included on the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not completed and submitted with all the required information . San <br /> Joaquin County Environmental Health Department(SJC EHD/CUPA) phone number was not listed in the <br /> emergency response/contingency plan.The locations of the emergency equipment section was not filled out listed <br /> in the emergency response/contingency plan. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD)at <br /> (209)468-3420 <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0010093 PR0520068 SCO01 10/26/2018 <br /> EHD 19-01 Rev.09/22/16 Page 2 of 4 Hazardous Materials Program OIR <br />
