The following is an itemized list of hazardous materials violations that have not been <br />addressed for SPORTSMANS WAREHOUSE as of Apri116, 2018. <br />Open violations from January 04, 2018 inspection <br />Violation #2 - Failed to complete and electronically submit a hazardous materials business plan. <br />The facility's business plan was not submitted electronically to the California Environmental Reporting System <br />(CERS). As of January 1,2013, all businesses were required to submit all business plans electronically. <br />Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br />the EHD for approval. <br />Violation #3 - Failed to establish and implement a hazardous materials business plan. <br />A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br />Plan shall be established and implemented for emergency response to a release or threatened release of a <br />hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br />materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br />pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />enter the required information, and submit for review by the EHD. <br />Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br />site. <br />Reportable amount of propane was found on site that has not been reported. Any material that meets or exceeds <br />the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br />inventory, and submit to the EHD for approval. <br />Violation #6 - Failed to complete and/or submit a site map with all required content. <br />A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br />orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br />emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br />response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br />"NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />upload the correct or updated information, and submit to the EHD for approval. <br />Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures have not been submitted. A business plan shall include the following <br />emergency response procedures for a release or threatened release of hazardous materials, including, but not <br />limited to, the following: <br />Immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- Evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />Violation #8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br />A training program has not been submitted. Training programs for all employees and annual training, including <br />refresher courses, shall include safety procedures for the event of a release or threatened release of a hazardous <br />material, including, but not limited to, familiarity with the facility's emergency response plans and procedures. These <br />training programs may take into consideration of each employee. Immediately prepare or update the facility's <br />training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />upload the correct or updated information, and submit to the EHD for approval. <br />Page 1 of 2
