The following is an itemized list of hazardous materials violations that have not been <br />addressed for SPORTSMANS WAREHOUSE as of Apri116, 2018. <br />Open violations from January 04, 2018 inspection <br />Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Employee training records were not available at the time of inspection. The business plan shall include provisions <br />for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br />documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br />provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br />Violation #12 - Failed to have the business plan readily available to facility or EFID personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br />plan readily available for review. <br />Page 2 of 2
