SAN 10 A Q U I N Environmental Health Department <br /> COUNTY— <br /> Aboveground Petroleum Storage Act Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Global Materials Management, LLC 1640 N Broadway Ave, Stockton August 26, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 620 CFR 112.7(f)(2) Failed to designate a person accountable for discharge prevention. <br /> This facility does not have a designated person for discharge prevention. The Spill Prevention, Control, and <br /> Countermeasure (SPCC) Plan shall designate a person at the facility who is accountable for discharge prevention <br /> and who reports to facility management. Immediately designate a person to be accountable for discharge <br /> prevention and update the SPCC Plan to include this information. <br /> This is a Class II violation. <br /> 622 CFR 112.7(f)(3) Failure to conduct spill prevention briefing for oil-handling personnel. <br /> Discharge prevention briefings are not conducted. Discharge prevention briefings for oil handling personnel must <br /> be scheduled and conducted at least once a year to assure adequate understanding of the SPCC Plan for that <br /> facility. Such briefings must highlight and describe known discharges or failures, malfunctioning components, and <br /> any recently developed precautionary measures. Immediately schedule and conduct a discharge prevention <br /> briefing, and ensure that they are scheduled and conducted at least once a year. <br /> This is a minor violation. <br /> 706 CFR 112.8(c)(2) Failed to provide and maintain adequate secondary containment. <br /> The oil storage drums and compactor were observed with insufficient secondary containment. No secondary <br /> containment was observed for the 180+ drums of used oil or the used oil storage in the filter compactor. All bulk <br /> storage tanks must be provided with a secondary means of containment for the entire capacity of the tank and <br /> sufficient freeboard to contain precipitation. Immediately provide sufficient secondary containment for this and all <br /> other tanks at this facility. <br /> This is a Class II violation. <br /> 712 CFR 112.8(c)(6) Failed to test or inspect each container for integrity based on industry standards. <br /> Integrity testing was not conducted on the aboveground oil storage containers on site. Each aboveground container <br /> shall be tested and inspected for integrity on a regular schedule and whenever repairs are made. The qualifications <br /> of personnel performing tests and inspections, frequency and type of testing and inspections that take into account <br /> container size, configuration, and design shall be determined in accordance with industry standards. Examples of <br /> these integrity tests include, but are not limited to: visual inspection, hydrostatic testing, radiographic testing, <br /> ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison records and <br /> other records of inspections and tests must be maintained on site. Immediately conduct the necessary testing and <br /> submit a copy of the test results to the EHD, or provide equivalence as allowed by CFR 112.7(a)(2). <br /> This is a Class II violation. <br /> 715 CFR 112.8(c)(8)(v) Failed to regularly test liquid level sensing devices to ensure proper operation. <br /> Liquid level sensing devices have not been tested. Procedures and frequency of testing for these devices were not <br /> addressed in the Spill Prevention, Control, and Countermeasure (SPCC) plan. Liquid level sensing devices must be <br /> installed in accordance with CFR 112.8 and shall be regularly tested to ensure proper operation.
