(See also items #18 19 27 & 27 below) Thelan must be amended when there is a change in the facility design, construction, operation, ornce that materially affects its potential for a discharge, within 6 months of the change, andnted as soon as possible, not later than 6 months following preparation of the amendment.tely make all necessary amendments to the SPCC Plan to accurately represent the proceduresies currently in place at the facility and submit a copy to EHD by August 31, 2010. <br /> 18. The facility diagram in the SPCC Plan is outdated and does not include all the above ground tanks <br /> and containers. The Plan shall include a facility diagram which must mark the location and contents of <br /> each fixed storage container and the storage area where mobile or portable containers are located. It <br /> must identify the location of and mark as "exempt" underground tanks. It must also include all transfer <br /> stations and connecting pipes, including intra-facility gathering lines. Immediately update the facility <br /> diagram to include all of the required information and submit a copy to EHD by August 31, 2010. <br /> 19, 21 & 27. The inventory and discharge & drainage controls listed in the SPCC Plan does not include <br /> the approximately 100-gallon gas tank in the storage area north of the office building, and the <br /> approximately 300-gallon gas tank (currently empty) on a mobile trailer at the northwest edge of the <br /> property. The SPCC Plan also listed the 80-gallon tank to hold motor oil, when the tank owner stated <br /> that it is holding diesel during this inspection. Discharge & drainage controls and potential equipment <br /> failure are not addressed for any of the unlisted containers. The SPCC Plan must include the correct <br /> type of oil and storage capacity of each fixed storage container, and either the type of oil and storage <br /> capacity of each portable container or an estimate of the potential number of portable containers, types <br /> of oil, and anticipated storage capacities. The Plan must include discharge or drainage controls, such as <br /> secondary containment around all containers and other structures, equipment, and procedures for the <br /> control of a discharge, and a prediction of the direction, rate of flow, and total quantity of oil which could <br /> be discharged as a result of each type of major equipment failure. Immediately update the inventory and <br /> discharge information in the SPCC Plan to accurately represent the petroleum storage at the facility. <br /> 24. Local CUPA (SJC EHD) phone number is not on the contact list of the SPCC Plan, and name and <br /> phone number of CaIEMA (formerly State CIES) not updated. The SPCC Plan must include a contact list <br /> and phone numbers for the facility response coordinator, National Response Center, cleanup <br /> contractors with whom you have an agreement for response, and all appropriate federal, state, and local <br /> agencies (CalEMA, US EPA, CUPA, Fire and Police, etc.) who must be contacted in case of discharge. <br /> Immediately update the contact list to include the most current version of the required information. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACI PTY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inctor: Remved By: / Title: <br /> Ma <br /> EH -02-003/e <br /> R /10 CONTINUATION FORM <br />
