Since it could not be demonstrated how long this waste was stored on site, immediately dispose of this <br /> j waste and submit a copy of the disposal receipt to the EHD. <br /> This is a Class II violation. <br /> 715 CCR 66273.39 Failed to keep complete records of each shipment of universal waste for three years. <br /> Copies of universal waste disposal records for 2012, 2013, 2014 and 2015 were not found on site. Universal waste <br /> shipments are going to the Livermore facility for disposal according to Kenneth Newman Universal waste disposal <br /> records shall be retained for three years and have them readily available for review. Immediately locate a copy of all <br /> universal waste disposal receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> ***THIS IS THE FULL OFFICIAL INSPECTION REPORT*** <br /> The inspection report was completed on October 28, 2015. This full official inspection report supersedes the <br /> checklist that was left on site with John Scott on October 19, 2015. One added violation, item #401, is included in <br /> this inspection report that was not reflected on the checklist that was left on site. The inspection lasted for 5 days <br /> and included the following dates: <br /> September 30, 2015 <br /> October 1, 2015 <br /> October 5, 2015 <br /> October 15, 2015 <br /> October 19, 2015 <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork within 30 days of receipt of full official inspection report. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
