SAN JOAQUIN <br />COUNTY <br />Environmental Health Department <br />g. Verification of potential leaks from piping and fuel filters drain directly into UDC's. <br />h. Verification of dispenser footprints in the UDC's. <br />i. Verification of the installation of the Bravo CONY -132000 conversion frame per <br />manufacturer instructions. <br />j. Bravo Dispenser Conversion Evaluation Form shall be onsite at the time of inspection. <br />Please be aware that the facility cannot operate the UST system or receive fuel in the 87, <br />91 and diesel tank prior to the EHD inspection and approval. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />to the installation company), or a Tank Tester's license issued to the individual by the State <br />Water Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service Technician <br />certificate issued by the International Code Council (ICC), and <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />5. Individuals installing underground storage tank system components must possess or work <br />under the direct and personal supervision of an individual physically present at the work site <br />who possesses all of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, and <br />b. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br />Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br />installed. <br />6. Log into the California Environmental Reporting System (CERS), update and submit the "UST <br />Monitoring Plan" for affected tanks for approval. <br />7. Log into the California Environmental Reporting System (CERS), complete and submit a "UST <br />Certification of Installation/Modification" form for the affect UST components within 30 days. <br />8. All components must be certified by an independent testing organization and labeled as <br />required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br />Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br />9. Permit expires 180 days from the date of approval.
