Please see attached work order dated 5/21/14. <br />Q Designated Operator will continue to perform a visual inspection and will address <br />issues if deficiencies found. <br />#405. The 87 vent line vacuum liquid sensor (S2) and the 87/91 vapor recovery line vacuum <br />(S3) failed when tested. The continuous monitoring system shall be maintained to detect <br />the entry of the liquid or vapor phase of the hazardous substance stored in the primary <br />containment into the secondary containment and to detect water intrusion into the <br />secondary containment. <br />Q The float on the 87 vent line vacuum liquid sensor (S2) was replaced during the <br />inspection. <br />u The 87/91 vapor recovery line vacuum (S3) was retested and passed on 5/16/14. <br />U Please see attached Monitoring Certification. <br />#201. According to the designated operator inspection report, alarms on April 25, 2013 was <br />due to a technician on site. No maintenance records were found on site for this date. <br />u Chevron contractor (SSS) was on site on 4/25/13 due to the Diesel line leak detector <br />repairs.
