Report completed on May 8, 2014. Checklist left on site at <br />time of inspection. Added violation #303. <br />Class II violations: (303, 404, 405, 406) <br />303 & 406. The liquid level in the annular space of UDCs 5/6 and 9/10, as measured from the inside top <br />of the UDC to the level of liquid in the reservoir, was low and not capable of monitoring the complete <br />annular space. The liquid levels were as follows: <br />UDC 5/6: 8 inches below <br />UDC 9/10: 1/2 inch below <br />The UDC annular space liquid sensors and the vent sump annular space liquid sensor were not located <br />at the top of the annular space, to detect a drop in annular space liquid at the earliest opportunity. The <br />sensors were located the following distances below the inside top of the sumps: <br />UDC 1/2: 5 inches below <br />UDC 3/4-9 inches below <br />UDC 5/6: 9 inches below <br />UDC 7/8: 5 inches below <br />UDC 9/10: 41/2 inches below <br />UDC 11/12: 6 inches below <br />Vent sump: 3 inches below <br />All monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. The sensors monitoring the liquid level in the interstitial space shall be maintained so that a <br />breach in the primary or secondary containment is detected before the hazardous substance is released <br />into the environment. Correct immediately by having a properly licensed, trained, and certified <br />contractor replace the failed component with a functional component (LG 113-x listed, if applicable). <br />Obtain a permit within one business day from the EHD, if necessary.
