Diesel fuel was observed today in the under dispenser containments 3 and 11. Any release from <br />the primary containment that does not escape the secondary containment and was cleaned up within <br />eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EHD by May 24, 2013. <br />303. The Diesel fill sump liquid sensor was located approximately six inches off the lowest point of the <br />sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be <br />maintained to be able to detect a leak at the earliest possible opportunity. The technician placed the <br />liquid sensor at the lowest point of the sump. Violation resolved. <br />311. The Diesel line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br />obtain a permit within one business day from the EHD. If the failed leak detector can't be repaired <br />immediately, there is a possibility that the Diesel UST system may be red tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILP IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
