Violations Effective immediately, all containers holding <br />#2 & #5: hazardous waste shall be closed except when adding <br />or removing waste. <br />Violation #6: Within 10 days of receipt of this report, adequate <br />spill control and decontamination equipment shall <br />be available at hazardous waste storage areas. <br />Violation #7: Within 14 days of receipt of this report, all <br />drums containing hazardous waste which have been <br />stored in excess of 90 days shall be transported <br />by a California Registered Hazardous Waste Hauler <br />to an approved Treatment, Storage or Disposal <br />Facility. <br />Violation #8: Within 10 days of receipt of this report, employee <br />training records which document on-the-job <br />training to ensure compliance with these <br />regulations shall be available for inspection. <br />Violation #9: Within 10 days of receipt of this report, records <br />indicating job title for each position at the <br />facility related to hazardous waste management and <br />the name of the employee filling each job shall be <br />available for inspection. <br />Violation #10: Within 10 days of receipt of this report, a <br />written job description for each position <br />listed under violation #8 [Section 66265.16 (d) <br />(1)] shall be available for inspection. <br />Violation #11: Within 30 days of receipt of this report, submit a <br />biennial report to the State Department of Toxic <br />Substances Control and maintain a record of the <br />submittal on site. <br />When all violations have been corrected, please send written <br />certification to this office at the letterhead address. This <br />certification shall contain the sample certification statement <br />indicated on Attachment I. <br />PHS-EHD may conduct a follow up inspection of the facility to <br />verify compliance. <br />The issuance of this report of violations and schedule of <br />correction does not preclude PHS-EHD from taking any <br />administrative, civil, or criminal action as a result of the <br />violations noted herein. <br />
